Sure they do, that's forged stuff though and forged pistons are not being discussed here. Also: having a perfect piston in the game doesn't make sense for gameplay.

But not is an perfect piston, is an more realistic piston, or unless, the option in the game to make custom aftermarket parts.

The game wont do aftermarket stuff. If they have to, the game wont be finished in another 10 years.
If you really want them, you can mod them in after the final release.
